Historically, Home and Community Care (HCC) has been associated with seniors services like adult day programs, Meals on Wheels and transportation assistance.
Today, Home and Community care is much more.
Over the years, it has evolved to meet the changing needs of clients, and now comprises more than 25 health and wellness services delivered by a range of medical and non-medical professionals and volunteers, providing acute and complex care customized to meet the needs of a diverse group of more than 1 million Ontarians.
Many people imagine HCC is strictly for well seniors, or post-acute care patients, however these services benefit a wide spectrum of Ontarians, including:
HCC programs are enhanced by volunteers, who donate over 3 million hours each year, saving the health care system over $80 million in paid services annually.
